I authorize Resting Waves LLC to provide sea scattering services

I declare that I have legal right and authority to autherize the dispostion of the decedent's cremains

I understand once the cremains are scattered they cannot be retrieved

I release the cremains of the named decendent for this service

Unless otherwise stated, the cremains container will be disposed

I agree to release Resting Waves LLC of all legal action in the event the cremains are lost or damaged during shipping

All cancellations must be made prior to the scattering of the cremains. Cancellations will receive a full refund minus a 10% processing fee and the cost of return shipping
